The transition pathway shows the journey through transition, to help and support young people and their families prepare for the future.

The Transition Team

The City of York’s  Multi-Agency Team supporting disabled young people and those with Special Educational Needs as they become adults

Personalised Learning and Work Package

The Personalised Learning and Work Package is your chance to choose what activities you can do when you leave school and live in York.

York Independent Living and Travel Skills or YILTS

This is a travel training programme for students with a range of special educational needs across the City of York Council area.

YILTS offer training to young people whilst they are at school. Carefully planned programmes support young people to understand timetables, plan routes and understand what to do when something unexpected happens.

United Response are able to offer travel training to students once they have left school – for further information please contact your Care Manager.
